ERROR 500 - MediaCacheItemBase.saveBlock : java.lang.NullPointerException

On a pair of Edge servers I see that a few files got “stuck” in the media cache folders, and this error occurs

The files were weeks old, though the time to live was a most 20 minutes.

Any ideas?
